I recommend that a Facility Manager/Director interview their stakeholders (e.g. such as management and department heads) annually; ideally in preparation for budget season. The intent is not necessarily for the FM to hand the survey to the stakeholder to have complete it; rather, it is used by the FM to provide talking points and to document feedback.
The FM should get the stakeholder to not think about only facility related issues; rather the FM should probe to learn more about the stakeholder’s business drivers, obstacles, constraints, etc. This will help the FM to gain better insight on how the Facility Department can best support and serve their clients.
